depends on where you live and there would be no reason to have two coverages as most companies forbid this anyway.

1) It depends on where you live.
2) No the premium does not apply to the deductible, neither do copays.
3) Very rarely. Why would you want to?
4) Yes. Preexisting conditions.

Try this one - - my family have their health insurance - it is affordable and has good coverage for dental issues.
